Change Your PCP

How to transfer your insurance to us

Want to make Apple Medical your primary care provider (PCP)? Using UnitedHealthcare (UHC) as an example, there are two ways:

Method 1: Change your PCP online

  1. Go to myuhc.com and click “Register Now.” Enter your details exactly as they appear on your UHC member ID card — name, date of birth, member ID, and ZIP code — then create a username and password, set your security questions, agree to the terms, and verify your email to activate the account.
  2. Log in to your member portal at myuhc.com, navigate to the “Primary Care Physicians” tab, and select “Change PCP.”
  3. Enter Dr. John Lee’s NPI (National Provider Identifier) number 1932983236 and submit the request.
  4. You will receive a new member ID card with Dr. John Lee as your PCP within about 2 weeks.

Method 2: Change your PCP by phone

  1. Have your insurance card ready and call the member services number on the back of the card; Chinese-language service is usually available if you need it.
  2. Say that you would like to change your primary care doctor; when asked which doctor, give them Dr. Lee’s NPI number 1932983236.
  3. If asked why you are changing, you can simply answer: it is more convenient.

Are physicals and vaccinations billed separately?
No. They are included in the visit fee.
